Patient Portal
The patient portal allows patients to enter demographic information, medical information, and acknowledge required documents online through a secure login. It allows information to be exchanged securely and in full compliance with HIPAA.
The Patient Portal allows patients to provide information from the comfort of their own home and reduce time during their office visit.

The patient portal also allows new and established patients to request appointments online!
Data collection: We will collect your name, email address, mailing address, and mobile phone number when you sign up for SMS updates. The information will be collected via the website contact form, email, or in some cases in person. Data usage: We will use data solely for scheduling appointments, changing appointments, and sending out appointment reminders.
Data security: We will protect your data with secure storage measures to prevent unauthorized access.
Data retention: We will retain information as long as you are subscribed to our SMS service. You may request a deletion at any time.
Message and data rates may apply. Your mobile carrier may charge fees for sending or receiving text messages, especially if you do not have an unlimited texting or data plan.

Patient Portal Information
The following information will be needed when registering on the patient portal: Name, Date of Birth, Social Security Number, Sex, Address, Phone. Patients must have a valid email address and choose a password. The patient policies must be accepted to create a portal account.

You can always log into the portal to update your demographic and medical information.
How do I make an appointment?
Once logged in to the portal, scroll down to the communications section and click on "request appointment"

The information entered into the portal is secure and HIPAA compliant.
